中医药治疗心力衰竭作用机制的研究进展

摘要  中医药可改善心力衰竭病人症状,提高生活质量,降低再住院率和全因死亡率。中医通过抗心肌缺血、心肌凋亡、心肌纤维化、炎症反应、氧化应激、肾素.血管紧张素.醛固酮系统以及促进血管生成、调节能量代谢和肠道菌群等,发挥抗心力衰竭作用。综述治疗心力衰竭的中药新药进展,对中医药治疗心力衰竭作用机制进行探讨,以期为心力衰竭的研究提供借鉴。

慢性心力衰竭在世界范围内的流行正在扩大,治疗心力衰竭是心血管领域的巨大挑战,亦是医学现代化面临的难题  [1.3] 。数千年来,中医药一直广泛应用于心力衰竭的临床治疗,独具优势  [4] ,辨证的个体化用药  [5] ,多靶点整体治疗  [6.7] ,缓解胸闷、气短、乏力、水肿等症状  [8.9] ,提高生活质量,增强免疫力,降低药物毒副作用  [10] 等。近年来,医学家们开展了广泛的研究探索中药治疗心力衰竭的作用机制。现对治疗心力衰竭的中药新药进展及作用机制进行总结并探讨。

1 治疗心力衰竭中药新药

中医与现代科技的结合使其发挥新优势,中医药治疗心力衰竭的研究正在大规模地展开,证据基础不断扩增,加快了中医治疗心力衰竭的发展。在中医理论指导下挖掘中医经典方药,从生物学、药理学、计算机科学及现代工艺等多科学研究,促使中医药治疗心力衰竭取得新突破。基于中医经典名方、名医经验、中医新理论、膏方理论、现代药理研究及新制作工艺等,从中医学宝库中挖掘出防治心力衰竭的中药新药,品类多样、功效独特、适用范围广泛,包括胶囊剂、丸剂、片剂、膏剂、口服液剂和静脉制剂等,显著提高了慢性心力衰竭病人的生活质量,改善心功能和临床症状,降低病死率。详见表1。心力衰竭慢性期除丸散外还可选用膏剂,防治结合,口感颇佳,病人依从性好,可长期服用  [11] 。静脉制剂多用于心力衰竭急性加重期,研究表明,联合中药效果优于纯西医治疗  [12] ,如参麦注射液、生脉注射液、参附注射液在降低N末端脑钠肽前体(NT.proBNP)、脑钠肽在提高临床总有效率方面具有优势;参麦注射液、丹红注射液、生脉注射液在提高心脏泵血功能方面更具优势;参麦注射液、生脉注射液、益气复脉注射液在总体疗效及改善单项指标方面均有明确优势。

2 中药治疗心力衰竭的作用机制

心脏负荷过载后出现心室扩大、心肌肥厚、心肌收缩力及心室顺应性下降等,导致并加速心室重塑。因此,保护心肌功能和延缓心室重塑是中药治疗心力衰竭的重要研究方向。中药通过抗心肌缺血、心肌纤维化、炎症、氧化应激、凋亡,促血管生成,抑制肾素.血管紧张素.醛固酮系统(RAAS),调节能量代谢及肠道菌群等多维途径改善心室重构,进而发挥抗心力衰竭的作用。详见表2。芪参益气滴丸调节心肌纤维化  [27] 、氧化应激和能量代谢  [28] 、RAAS  [29] 、血管生长  [30] 、炎症  [31] 、凋亡  [32] 等;四逆汤  [33.35] 改善心功能、抗纤维化和炎症、抑制RAAS;芪苈强心胶囊  [36.38] 抗纤维化、凋亡、炎症和心肌肥大等。

中医学认为“心与小肠相表里”,调节肠道功能可作为治疗心血管疾病的新靶点,心肠轴可能成为今后中医治疗心力衰竭的研究方向  [39] 。肠道微生物代谢物包括三甲胺、胆汁酸、益生菌和微生物酶等,通过细菌易位等多种代谢途径影响人体代谢及心脏功能,发挥治疗心力衰竭的作用  [40.41] 。参附注射液通过增加短链脂肪酸产生菌改善因心力衰竭后大鼠紊乱的肠道细菌结构,促进肠道有益细菌的生长,进而改善心力衰竭  [42] 。补阳还五汤通过降低心力衰竭大鼠血清氧化三甲胺(TMAO)含量,改善肠道菌群,延缓心力衰竭进展  [43] 。总之,基础实验研究在不同的动物心力 衰竭模型中验证了中药通过多靶点、多途径发挥治疗心力衰竭的作用,但心力衰竭具体证型的作用机制仍需进一步深入探讨。

3 小结与展望

中医药治疗心力衰竭的临证疗效颇佳,但仍面临药物成分繁杂、发挥作用的物质基础不明确、基础实验机制的研究进展缓慢、系统的机制研究缺乏等问题,亟需开展高质量的研究。心力衰竭病机复杂,证型多样,脏腑气血阴阳之本虚,瘀血、痰浊、水饮、气滞之标实,本虚标实之象缺乏切合的机制研究。各种局限性因素影响中医治疗心力衰竭研究的实际利弊结论,是中医治疗心力衰竭的薄弱环节,制约了中医治疗心力衰竭的发展和推广。尽管治疗心力衰竭的中药研究面临着较大挑战,但借助于现代科学技术,在中医理论指导下,进一步探讨中药治疗心力衰竭的作用机制,深入挖掘中药治疗心力衰竭不同病机、证型的生物学内涵,系统阐明方药及其有效组分的药理机制,可能在心力衰竭治疗研究中获得新突破,展现中药治疗心力衰竭的综合优势,从而加快推进有效方药的应用和推广,给予心力衰竭更好的治疗支持。
