单纯内固定与内固定联合椎体成形术治疗脊柱骨折的效果对照研究

[摘要] 目的 對比分析脊柱骨折患者采用单纯内固定、内固定联合椎体成形术治疗的效果。方法 方便选取该院2017年4月—2019年4月收治的脊柱骨折患者70例，随机分为两组，各35例，对照组35例行单纯内固定治疗，观察组35例行内固定联合椎体成形术治疗，对比两组手术治疗效果。结果 观察组治疗优良率显著高于对照组（94.29% vs 71.43%）（χ2=6.437，P<0.05）;观察组和对照组的术中失血量和住院时间分别为（57.63±12.81）mL、（8.68±1.36）d，均显著少于对照组（t=74.002、10.660，P<0.05），观察组手术时间为（94.36±10.56）h，显著多于对照组（t=2.731，P<0.05）;观察组治疗后的Cobb's角、伤椎高度为（17.33±3.10）°、（1.85±0.41）cm，与对照组相当（t=0.367、0.202，P>0.05）;观察组椎体后缘高度压缩率和椎体前缘高度压缩率分别为（12.33±3.65）%、（3.02±0.58）%，均显著少于对照组（t=6.092、18.809，P<0.05）;观察组术后并发症发生率显著少于对照组（2.86% vs 11.43%）（χ2=5.938，P<0.05）。结论 内固定联合椎体成形术治疗脊柱骨折效果优于单纯内固定手术。

[Abstract] Objective To compare and analyze the effects of simple internal fixation， internal fixation combined with vertebroplasty in patients with spinal fractures. Methods A total of 70 patients with spinal fractures admitted to the hospital from April 2017 to April 2019 were convenient selection randomly divided into two groups， 35 cases in each， 35 cases in the control group were treated with internal fixation alone， and 35 cases in the observation group were treated with internal fixation vertebroplasty treatment， comparing the effects of surgical treatment between the two groups. Results The excellent and good rate of treatment in the observation group was significantly higher than that in the control group （94.29% vs 71.43%） （χ2=6.437， P <0.05）; the intraoperative blood loss and hospital stay in the observation group and control group were （57.63±12.81） mL， （8.68±1.36） d， were significantly less than the control group （t=74.002， 10.660， P<0.05）， the operation time of the observation group was （94.36±10.56）h， significantly more than the control group （t=2.731， P<0.05）; Cobb's angle and injured vertebral height of the observation group after treatment were （17.33±3.10） ° and （1.85±0.41） cm， which was equivalent to the control group （t=0.367， 0.202， P> 0.05）; observation group posterior vertebral body height compression rate and the anterior vertebral body edge compression rate were （12.33±3.65）% and （3.02±0.58）%， respectively， which were significantly less than the control group （t=6.092， 18.809， P<0.05）; observation group of the postoperative complication rate was statistically significantly less than the control group （2.86% vs 11.43%）（χ2=5.938， P<0.05）; Conclusion The effect of internal fixation combined with vertebroplasty in the treatment of spinal fractures is better than simple internal fixation .

综上所述，对于脊柱骨折患者，给予内固定联合椎体成形术治疗效果优于单纯内固定手术治疗，具有创伤小、出血量少、住院时间短、并发症少的优点。
