[摘要] 目的 探討弹性钉闭合复位内固定治疗儿童桡骨颈骨折的疗效。 方法 收集2015年1月至2020年1月我院(珠海市妇幼保健院)收治的56例儿童桡骨颈骨折患儿的临床资料,按照手术治疗方式的不同分为对照组和研究组,对照组为采用切开复位克氏针固定术治疗的28例患儿,研究组为采用弹性钉闭合复位内固定疗法的另外28例患儿,记录两组患儿手术时间、术中出血量以及术后住院天数,并进行为期1年的随访,对其患肢功能、不同时间点的疼痛程度进行评估。 结果 对照组的手术时间、术后住院时间分别为(90.93±4.82)min、(11.38±2.73)d,均较研究组的(43.23±4.83)min、(6.97±1.86)d长;对照组术中出血量为(104.34±20.47)mL,较研究组的(50.38±6.93)mL多;对照组患肢功能优良率为85.71%,较研究组的100.00%低;术后不同时间点,对照组的视觉模拟评分法(VAS)评分均较研究组高,差异有统计学意义(P<0.05)。 结论 弹性钉闭合复位内固定治疗儿童桡骨颈骨折能够显著提高治疗效果,减轻患儿的疼痛感,改善肢体功能,值得推广。

The curative effect of closed reduction and internal fixation with elastic nails in the treatment of pediatric radial neck fractures

[Abstract] Objective To explore the curative effect of closed reduction and internal fixation with elastic nails in the treatment of pediatric radial neck fractures. Methods The clinical data of 56 children patients with radial neck fractures who were admitted to our hospital (Zhuhai Women and Children's Hospital) from January 2015 to January 2020 were collected. According to different surgical treatment methods, the patients were divided into the control group and the study group. The control group of 28 children patients was treated with open reduction and Kirschner wire fixation. The study group of 28 children patients was treated with elastic nails for closed reduction and internal fixation. The operation time, intraoperative blood loss, and postoperative length of hospital stay were recorded for both groups, and they were followed up for a year. The function of the affected limb and the degree of pain at different time points were evaluated. Results The operation time and postoperative length of hospital stay in the control group were (90.93±4.82) min and (11.38±2.73) days, respectively, which were longer than those of (43.23±4.83) min and (6.97±1.86) days in the study group; the intraoperative blood loss in the control group was (104.34±20.47) mL, which was more than that of (50.38±6.93) mL in the study group; the excellent and good rate of limb function in the control group was 85.71%, which was lower than that of 100.00% in the study group; at different time points after operation, the visual analogue scale (VAS) scores in the control group were higher than those in the study group, and the differences were statistically significant (P<0.05). Conclusion The closed reduction and internal fixation with elastic nails for pediatric radial neck fractures can significantly improve the curative effect, reduce the pain of the children patients, and improve the function of the limbs, which is worthy of promotion.
