Efficacy of phacoemulsification combined with goniosynechialysis and trabeculectomy on acute angle-closure glaucoma complicated with cataract

[Abstract] Objective To study the efficacy of trabeculectomy,phacoemulsification and goniosynechialysis in the treatment of acute angle-closure glaucoma (AACG) complicated with cataract. Methods The medical records of 107 patients with AACG complicated with cataract confirmed in our hospital from January to December 2019 were analyzed retrospectively. Based on the difference of surgical methods,54 cases were classified into the observation group (phacoemulsification and goniosynechialysis)and 53 cases were classified into the control group (trabeculectomy). The visual acuity,intraocular pressure,anterior chamber depth and surgical complications were compared between the two groups. Results The best corrected visual acuity (BCVA) was (0.42±0.13)in the observation group and (0.41±0.11)in the control group before operation,with no statistically significant difference (P>0.05). After operation,the BCVA of the observation group was (0.20±0.04),which was lower than that of the control group (0.36±0.06), with statistically significant difference (P<0.05). The BCVA in each group of the two groups was lower than that before operation,with statistically significant difference (P<0.05). Before operation, the intraocular pressure was (18.31±4.47) mmHg in the observation group and (18.36±4.52)mmHg in the control group, with no significant difference (P>0.05). After operation,the intraocular pressure in the observation group was (15.45±2.03) mmHg,which was lower than that in the control group (17.23±1.38)mmHg,with statistically significant difference (P<0.05). The postoperative intraocular pressure in each group of the two groups was lower than that before operation,with statistically significant difference (P<0.05). The anterior chamber depth before and after operation in the observation group was (2.24±0.20)mm and (3.49±0.31)mm,respectively, while the anterior chamber depth in the control group was (2.23±0.19)mm and (3.45±0.29)mm at the same time,with no statistically significant differences (P>0.05). However, the anterior chamber depth in each group of the two groups was higher than that before operation, with statistically significant difference (P<0.05).The incidence of complications in the observation group was 3.70%, which was lower than that in the control group (16.98%), with statistically significant difference (P<0.05). Conclusion Phacoemulsification and goniosynechialysis has good visual correction effect for patients with AACG complicated with cataract, with ideal intraocular pressure and high safety during perioperative period.
