中药熏蒸治疗糖尿病相关性干眼症的疗效观察

[摘要] 目的 探讨中药熏蒸治疗糖尿病相关性干眼症的效果。 方法 选择2017年1月～2018年1月在我院诊断治疗的糖尿病相关性干眼症患者60例为研究对象，随机分为研究组与对照组，各30例。对照组采用人工泪液治疗治疗，研究组在此基础上加用中药熏蒸治疗。比较两组治疗后主观症状评分、角膜染色、泪膜破裂时间、泪液分泌量评分及两组临床疗效。 结果 两组治疗后角膜染色评分显著低于治疗前，膜破裂时间、泪液分泌量评分显著高于治疗前，差异有统计学意义（P<0.05）;治疗后，治疗组角膜染色评分显著低于对照组，膜破裂时间、泪液分泌量评分显著高于对照组，差异有统计学意义（P<0.05）。两组治疗后主观症状得分均显著低于治疗前，差异有统计学意义（P<0.05）;治疗后研究组主观症状得分显著低于对照组，差异有统计学意义（P<0.05）。研究组总有效率显著优于对照组，差异有统计学意义（P<0.05）。 結论 中药熏蒸治疗糖尿病相关性干眼症能够显著改善患者症状，提高临床疗效。

[Abstract] Objective To discuss the effect of traditional Chinese medicine fumigation on diabetic associated xerophthalmia. Methods Sixty patients with diabetes-related xerophthalmia who were diagnosed and treated in our hospital from January 2017 to January 2018 were selected as the subjects and were randomly divided into study group and control group with 30 cases in each group. The control group was treated with artificial tears， while the study group was treated with traditional Chinese medicine fumigation. The scores of subjective symptoms， corneal staining， tear film rupture time and tear secretion were compared between the two groups. Results After treatment， corneal staining score was significantly lower than that before treatment， membrane rupture time， tear secretion score were significantly higher than those before treatment， the difference was statistically significant（P<0.05）. After treatment， the corneal staining score of the treatment group was significantly lower than that of the control group， and the membrane rupture time and tear secretion score were significantly higher than those of the control group（P<0.05）. The scores of subjective symptoms in the two groups after treatment were significantly lower than those before treatment（P<0.05）， and the scores of subjective symptoms in the study group after treatment were significantly lower than those in the control group（P<0.05）. The total effective rate of the study group was significantly better than that of the control group（P<0.05）. Conclusion Traditional Chinese medicine fumigation treatment of diabetes related dry eye can significantly improve symptoms of the patients and improve clinical efficacy.

干眼症是泪液质、量的异常，或者动力学异常，泪膜稳定性下降，眼部不适，导致眼表病变。我国糖尿病的发病率呈逐率上升的趋势，目前我国糖尿病患病人数居全球第二，仅次于印度[1]。高血糖水平可导致多种眼部并发症，例如白内障、视网膜病变、睑缘炎等[2]。糖尿病患者干眼症的发病率大大高于正常人群，与泪液生成不足有关，泪腺功能下降，黏蛋白缺乏，脂质缺乏，角膜知觉减退等[3]。研究显示干眼症严重程度与视网膜病变密切相关[4]。中药熏蒸治疗是以中医理论为指导，利用药物煎煮后产生的蒸汽，通过熏蒸机体达到治疗目的的一种中医外治方法[5]。本研究对糖尿病相关干眼症患者实施中药熏蒸治疗，取得了较好的效果，现报道如下。
