【摘要】 目的:对生长分化因子-15水平(growth differentiation factor-15,GDF-15)與急性冠脉综合征(acute coronary syndrome,ACS)患者心功能分级的临床意义进行研究。方法:58例ACS患者按纽约心脏病协会(NYHA)的心功能分级方法分为NYHA Ⅰ级组(21例)、NYHA Ⅱ级组(22例)、NYHA Ⅲ级组(10例)、NYHA Ⅳ级组(5例)四个亚组。25例行冠脉造影排除冠心病的NYHA Ⅰ级患者为对照组。检测各组B型利钠肽(B-type natnuretic peptide,BNP)等生化指标,运用ELISA法测定GDF-15水平。结果:与对照组相比,ACS患者各亚组的GDF-15、BNP水平显著高(P<0.01),且各组GDF-15、BNP水平随着心力衰竭严重程度的升高而上升(P<0.05),GDF-15、BNP水平均与心衰程度呈正相关(r=0.681、0.653,P<0.001)。且GDF-15、BNP两者间亦呈现正相关性(r=0.726,P<0.001)。结论:ACS患者的GDF-15水平升高显著,与BNP呈正相关,可对ACS患者的心功能进行较有价值的危险分层,利于判定ACS患者心力衰竭程度。

【Abstract】 Objective:To approach the clinical significance between the levels of growth differentiation factor-15(GDF-15) and the heart function classification in acute coronary syndrome(ACS).Method:58 ACS patients were divided into four sub-groups such as Ⅰ degree(21 cases),Ⅱ degree(22 cases),Ⅲ degree(10 cases),Ⅳ degree(5 cases) according to the New York heart association(NYHA).25 cases with Ⅰ degree of NYHA excluded coronary artery disease by coronary arteriography were treated as the control group.Biochemical indicators such as B-type natnuretic peptide(BNP) were tested in all groups.The levels of GDF-15 were determined by antibody-sandwich enzyme–linked immunosorbent assay(ELISA).Result:Compared with the control group,the analysis showed that the levels of GDF-15,BNP in ACS sub-groups were significantly increased(P<0.01),and the levels of GDF-15,BNP expression increased with the degree of heart failure increased(P<0.05),also the levels of GDF-15,BNP in ACS patients had a positive correlation with the degree of heart failure(r=0.681,0.653,P<0.001).Then the GDF-15 was positively correlated with BNP(r=0.726,P<0.001).Conclusion:The levels of GDF-15 in ACS patients are markedly increased,also has a positive correlation with BNP.They have more valuable to evaluate the risk stratification of the heart function,also can be used to judge the severity of heart failure in ACS patients.

急性冠脉综合征(ACS)是心血管内科常见的疾病之一,起病危急,是威胁着人类健康的多因素疾病。而ACS患者在疾病的后期常合并心力衰竭,这就使得患者的心功能、日常生活受到较严重的影响。如若能寻求较为特异的心血管生化指标来指导ACS早期诊断、危险分层,则对ACS的治疗、判定预后意义重大。研究发现,在创伤、炎症因子、缺血缺氧等刺激下,GDF-15的表达显著增加[1-4]。GDF-15与心血管疾病相关,其作为对抗心血管损伤的保护因子而发挥其保护作用[5-6]。可作为心血管功能、疾病的新兴标记物[7]。本文按照纽约心脏病协会(NYHA)的心功能分级方法对ACS患者实行相关分类,分析GDF-15水平与ACS患者心功能分级的意义,判断其是否可作为ACS患者心功能严重程度的标记物,评估ACS患者心力衰竭程度及其对ACS预后的价值。endprint
