长期吐舌习惯导致前牙开伴牙龈纵裂病例的治疗方法研究

目的：分析治疗因吐舌习惯造成的患者前牙开合并牙龈纵裂的临床有效措施。方法：收治前牙开合并牙龈纵裂患者12例，在相应的检查后制定针对性的矫形方案，然后应用直丝弓矫治器进行矫正，分析治疗效果。结果：患者经矫正治疗后，牙开消失，咬合关系恢复正常，治疗时长(21.98±7.92)个月，同时患者的牙周疾病得到改善9例。结论：经全面地诊断与针对性的矫形措施能有效地治疗前牙开疾病，恢复牙齿的正常咬合，同时经矫正过程中的正畸力可逐渐调整上颌骨与下颌骨的相对位置。

资料与方法

方法：对全部患者均在相应的模型、X线检查关节、测量头影后制定针对性的矫形方案，然后应用直丝弓矫治器进行矫正。首先，如患者存在后牙反，可应用活动扩弓器调节，对吐舌习惯应加舌屏纠正；如患者牙齿存在拥挤现象，可适当拔除中后位置的牙齿，对于患者的下颌牙弓则用固定矫齐。然后，对患者的上颌牙弓实施固定性地矫正，纠正偏离的颌中线位置，可将种植钉安于患者的颌骨颊棚部位进行适当辅助措施。对弓丝可选用适当规格的镍钛丝以及不锈钢丝。治疗结束，分析患者的治疗效果与治疗时长和颞下颌关节紊乱病情况。

综上所述，全面地诊断与采用直丝弓矫治器的针对性的矫形措施能有效地治疗前牙开疾病，恢复牙齿的正常咬合，同时矫正过程中的正畸力可逐渐调整上颌骨与下颌骨的相对位置，值得临床广泛应用。

Study on the treatment of patients with anterior open bite and longitudinal crack on the gum caused by long-term tongue thrusting

Objective:To analyze clinical effective treatment measures of patients with anterior open bite and longitudinal crack on the gum caused by long-term tongue thrusting.Methods:12 patients with anterior open bite and longitudinal crack on the gum were selected.We made targeted corrective action after appropriate inspection,and then used straight wire appliance to correct, then analyzed the therapeutic effect.Results:After orthodontic treatment,the anterior open bite disappeared and the occlusal relationship returned to normal,and the duration of treatment were(21.98±7.92)months;at the same time,periodontal disease was improved in 9 cases.Conclusion:A comprehensive diagnosis and targeted measures can be effective in the treatment of anterior open bite disease,restore the normal occlusion of the teeth,at the same time,the relative position of the maxilla and mandible can be adjusted by the orthodontic force.
