Abstract:Objective:TocloneandheterologouslyexpressAspergillusNigerUrateOxidaseinEscherichia coliinordertoproduceurateoxidaseinlargequantitiesformedicalpurposesMethods:Theresultsof genesequencingshowedthattheAspergillusNigerurateoxidasegenecontained909basepairsandwas encodedwith302aminoacidresidues.TheresultsofSDS-PAGEandnativegelelectrophoresisshowed thatthemolecularweightofmonomerenzymewas35~37kDa.Theproteinself-assembledintoatetramerin theexpressionprocess,andtheapparentmolecularweightwasabout140~150kDa. Results:The recombinantproteinwasexpressedincytoplasmofEscherichiacoliinalmostcompletelysolublestate, withtheoptimalpHfordissolutioninwaterbeingpH8.0andtheoptimaltemperatureforcatalyzing35℃. Thebacteriaafterinducedexpressionwerelysedintheoptimalbufferattheratioof1∶20(W∶V). Theactivityofthecrudeenzymewas67.69IU/mLandthespecificactivitywas47.00IU/mgafter ultrasonication.Conclusion:A new methodhasbeenprovedfortheurateoxidaseproductionfrom microorganism. Objective:Toexploredetectionsfortheearlyinfectedpersonsofschistosomajaponicumby meansofsoundspeedtissuequantification(STQ)togetliverzonespeedindex(ZSI)andthen obtainzonesoundspeed(ZSS). Methods:WithSTQ,ZonareZoneUltrahadbeenappliedto detecthepaticfibrosisinthe39casesofzero-levelschistosomiasis. TheZSIvaluesat0cmor1cm belowlivercapsuleintheleftlobe,andat0,1,2or3cmintherighthadbeendetected.Theachievementrate ateachpositionhadbeencalculatedwithintra-groupcorrelationcoefficient(ICC)toselectsuitablepositionsfor detection. Therelationshipbetweendepth,ageorsexandtheZSSvalueshadbeenanalyzed.Results: MeasuringliverZSIwithSTQwasofhighstability.TheZSIvaluesofschistosomiasisliverwerenot correlatedwithageandsex. Theachievementrateat1cmbelowlivercapsuleofrightlobewasthe highest.Conclusion:ZSImeasuredwithSTQcanbeusedtoassesstheschistosomiasisliverelasticity quantitatively. Objective:Toevaluatethedegreeofhepaticfibrosisinschistosomiasisjaponicabycontrastenhancedultrasoundtechnique(CEUS)Methods:40casesofclinicallydiagnozedschistosomiasishad beendetectedwithCEUS.Thetransittimeofhepaticartery-hepaticvein(HA-HVTT),portalveinhepaticvein(PV-HVTT)andhepaticparenchyma-hepaticvein(PA-HVTT)hadbeencalculated accordingtothetimewhentheultrasoniccontrastagenthadreachedthehepaticartery,portalvein,hepatic parenchymaandhepaticveinrespectivelyandthedifferencesandcorrelationhadbeenanalyzed.Results:The timewhenthecontrastagentreached HA-HVTT,PV-HVTTandPA-HVTTincreasedwiththebook=4,ebook=4severityofliverfibrosis,anditwasnegativelycorrelatedwiththedegreeofliverfibrosis. Conclusion: CEUSisofimportantclinicalsignificanceinevaluatingthedegreeofhepaticfibrosisinschistosomiasis. Objective:Toevaluatetheclinicaleffectsofserumprocalcitonin(PCT)onguidingantibiotic therapyforICU patientswithsevere multipletrauma Methods:70casesofsevere multipletrauma patientsintheCentralHospitalofJingzhouhadbeenchosenfrom81casesintotalandhadbeen randomlyassignedintotwogroups:thecontrolgroup(n=32)andtheexperimentalgroup(n=38). Inthecontrolgrouptheantibioticusehadbeenbasedonjudgmentandexperienceofphysicians.In theexperimentalgrouptheantibioticusehadbeenbasedonPCTconcentrationduringhospitalization.The timeofantibioticuse,costsofantibiotics,ICUhospitalizationdays,SOFAchangesintreatment,the incidenceofmultipleorgandysfunctionsyndromes(MODS),infectionandsepsisandpatientmortality within28dayshadbeenobserved.Results:Intheexperimentalgroup,thedurationofantibiotic therapywassignificantlyshorterthanthecontrolgroup[(12.0±5.31)dvs(16.6±7.8)d,P<0.05], andthecostsofantibioticweresignificantlylowerthanthecontrolgroup[¥(4931.9±2824.9)vs ¥(7004.3±3755.1),P<0.05]. Therewerenostatisticaldifferences(P>0.05)intheICU hospitalizationdays(12.08±9.72dvs12.72±8.58d),SOFAchanges(4.66±2.83vs4.88±2.79), theincidenceofMODS(6/38vs7/32),infection(22/38vs21/32)andthesepsis(13/38vs10/32), andmortality(3/38vs5/32)within28days. Conclusion:PCTcanbeeffectivelyusedforguiding antibiotictreatmentinseveremultipletraumapatientsintheICU,whichmaycontributetoshorteningthe durationofantibiotictherapyandreducingthecostsofantibiotic. Objective:Tostudythecharacteristicsofisletfunctionchangeinpatientswithincipient diabetesthroughfollow-upinvestigationonisletfunctionofruralpatientsinJingzhouDistrictMethods:220 casesofpatientswithprimarydiabetesmellitusinJingzhouDistricthadbeenselectedfor12months follow-up.Bloodsugarchanges,c-peptiderelease,insulinsecretionindexes(HOMA-beta)and insulinresistanceindexes(HOMAIR)hadbeenobservedrespectivelyafter3months,6monthsand 12months. Andthencharacteristicsofisletfunctionchangeinpatientshadbeensummarized. Results:①Bydrugtreatment,bloodsugarofthepatientswasuptostandardin3 months,no obviousfluctuationin6monthsand12months;②Duringfollow-up,c-peptidereleaseofthepatients decreasedobviouslyinthreemonthsaftertreatment,andtherewerestatisticallysignificantdifferencesin comparisonwiththereleasein6monthsand12months(P<0.05).③Duringfollow-up,insulin capacitydecreasedcontinuouslyaftertreatmentandreachedthelowestlevelinthreemonths,the differencebeingstatisticallysignificant(P<0.05);④Duringfollow-up,HOMA-βofthepatients continuedtofallwhileHOMA-IRcontinuedtorisewiththemostsignificantvariationsin3months (P<0.05).Conclusion:Withtheextensionofthediseasecourse,theisletfunctionoftheincipient diabeticpatientsgraduallydecreases,therebeingsignificantcorrelation.Andtheisletfunctionofthe patientscanreachthelowestin3months. Objective:ToinvestigatethevalueofgascontrastmediausedingastricMSCTscan.Methods:Gas andpurewaterhadbeenusedrespectivelyascontrastmediaingastricMSCTexaminationfor30patientswith gastriccancerwhohadbeendividedinto2groups.Fouraspectshadbeencomparedbetweenthe2book=5,ebook=5groups,includingacceptabilityofpatients,dilationdegreeofgastriccavity,contrastofdensity,and artifactnumbers. Results:Inthegasgroup,theacceptabilityofpatients,thedilationdegreeof gastriccavityandthecontrastofdensitywerebetterthanthoseinthepurewatergroupsignificantly, buttherewasnosignificantdifferenceinartifactnumbersbetweenthetwogroups.Conclusion:Gas hasadvantagesoverpurewaterinacceptability,gastriccavitydilationandcontrastofdensityandit canbeabettercontrastagentingastricMSCTexamination. Objective:Toinvestigatethe methodsforcontinuousqualityimprovementofmultiple drug-resistantbacteriamonitoringinB-levelhospitalsMethods:The2problemsin multipledrugresistantbacteria monitoringhadbeenanalyzed:low microbialspecimeninspectionrateandlow discoveryrateofmultipledrug-resistantbacteria. ThePDCA managementmodelhadbeenapplied fromJulytoDecember2012intheanalysisoftheabove2problemstoimprovemonitoringquality continuously.Results:Themicrobialspecimeninspectionraterosefrom14.60%inthefirsthalfof 2012to34.54%inthesecondhalfafterusingPDCA managementmodel(P<0.01);thediscovery rateofmultipledrug-resistantbacteriadecreasedfrom11.90%to8.33%(P<0.01);theincidence rateofmultipledrug-resistantbacteria(new MDROSseparationnumber/hospitalizationdaysofa thousandpatients)rosefrom0.51‰to0.67‰(P>0.05).Conclusion:PDCAmanagementcycleis agoodmethodtopromotethecontinuousqualityimprovementindrug-resistantbacteriamonitoring. Thestatisticalanalysishadbeenmadeviadescriptivestatistics andchi-squaretest.Results:Thetotalnumberoftheexaminedwas441,415detectedwithmaindiseasesor abnormalindicators(94.10%).Thedetectionratesofmaindiseasesorabnormalindicatorswerecompared inlightofgenderandages,andtherewerestatisticallysignificantdifferences(P<0.05).Conclusion:The staffhealthconditionisnotoptimistic,andhealthmanagementandguidanceshouldbestrengthenedfor thestaff. Di-(2-ethylhexyl)phthalate(DEHP),anenvironmentalendocrinedisruptor,actslike estrogenandwithantiandrogenicactivity,whichthehumanbodiesarewidelyexposedto.Studies findthatthemothersduringpregnancyandthefetusesareexposedtoDEHPmoreeasily,whichcan causeavarietyofdevelopmentaltoxicity. ThispaperreviewstheinfluencesofDEHPontheplacenta, fetaldevelopment.
