摘要:目的 探讨医护人员对重症监护室专业小组管理模式的体验。方法 采用质性研究中的现象学研究方法,于2019年5月对解放军第三〇五医院重症监护室全体医护人员进行半结构式的深度访谈,并采用Colaizzi现象学7步分析法对获得的资料进行归纳和整理,提炼出主题。结果 20名医护人员对重症监护室施行专业小组管理模式的体会主要有4个主题,分别为主题1科室工作更加规范化、精细化,使管理者横向管理科室工作;主题2有助于医护人员提升日常工作能力及沟通能力,进而降低医疗风险;主题3部分职责划分不明确,个别人员积极性有待增强;主题4专业小组管理模式人员分配欠妥。结论 开展专业小组管理模式,有利于调动重症监护室医护人员工作的积极性和参与性,但需处理好人员分配,明确职责分工,科室管理者应从问题出发,实施人性化、科学化的专业小组管理模式。

Abstract:Objective To explore the experience of medical staff on the management mode of the intensive care unit professional group.Methods The phenomenological research method in qualitative research was used to conduct semi-structured in-depth interviews with all in-patient medical staff in the 305 hospital intensive care unit in May 2019, and the data obtained by the Colaizzi Phenomenological 7-step analysis method were summarized to organize and refine the theme.Results The experience of 20 medical staff in the implementation of the professional group management model in the intensive care unit mainly has 4 themes, namely theme 1 department work is more standardized and refined, allowing managers to manage department work horizontally; theme 2 helps medical staff improve their daily routines Working ability and communication skills, thereby reducing medical risks; theme 3 part of the division of responsibilities is not clear, the enthusiasm of individual personnel needs to be enhanced; theme 4 professional team management mode personnel allocation is not appropriate.Conclusion The implementation of the professional group management mode is conducive to mobilizing the enthusiasm and participation of the medical staff in the intensive care unit. However, it is necessary to handle the personnel allocation and clarify the division of responsibilities. The department manager should proceed from the problem and implement the humanized and scientific professional group management mode.

重症监护室作为医院的重要组成部分,是医院抢救和治疗危重症患者的主要科室之一。入住患者病情危重、变化快,若不采取及时有效的治疗,则易引发各类并发症[1]。重症监护室仪器设备多、特殊治疗多、高危药品种类繁杂,应发挥最大效率,注重针对性、有效性和时效性,共同促进管理质量的不断提高。近年来,随着重症医学的迅猛发展,ICU护士面临的较大压力和心理负担[2]。如何在繁忙的工作中找寻规律,制定统一管理标准是ICU护理工作的重点之一。我院重症监护室自2019年1月1日起开始施行专业小组管理模式,本研究通过半结构式深度访谈法探讨重症监护室专业小组管理模式的运行及应用情况,旨在检验前期工作成果,加强医护人员沟通及协作能力,为提高科室管理效率奠定基础。
