【摘要】 目的:對实时超声弹性成像技术在梗阻性黄疸引起的肝脏病变中的诊断价值进行研究和分析。方法:选取2014年12月-2017年3月牡丹江医学院附属红旗医院和牡丹江医学院附属第二医院收治的86例梗阻性黄疸患者作为观察组,并按照总胆红素水平的高低将观察组患者具体分为轻、中、重度黄疸三组;同时以同期在牡丹江医学院附属红旗医院和牡丹江医学院附属第二医院接受健康体检的35例志愿者作为对照组。所有研究对象均接受实时超声弹性成像技术的检测,比较相关数据,分析实时超声弹性成像技术在梗阻性黄疸引起的肝脏病变中的诊断价值。结果:观察组患者的MEAN显著低于对照组,AREA%显著高于对照组,差异均有统计学意义(P<0.05)。轻度黄疸组、中度黄疸组、重度黄疸组的MEAN值均明显低于对照组(P<0.05),AREA%值均明显高与对照组(P<0.05);此外,观察组组内两两比较,差异均有统计学意义(P<0.05)。轻度黄疸组、中度黄疸组、重度黄疸组的不同b值时的肝脏ADC值均明显低于对照组(P<0.05);此外,观察组组内,随着黄疸程度的加重,患者肝脏ADC值呈逐渐下降趋势,组间两两间比较,差均有统计学意义(P<0.05)。结论:实时超声弹性成像技术是诊断梗阻性黄疸引起的肝脏病变的有效途径。

【Abstract】 Objective:To study and analyze the diagnostic value of real-time ultrasound elastography in hepatic lesions caused by obstructive jaundice.Method:86 patients with obstructive jaundice admitted to the Hongqi Hospital Affiliated to Mudanjiang Medical University and the Second Affiliated Hospital of Mudanjiang Medical University from December 2014 to March 2017 were enrolled in this study as the observation group,according to the level of total bilirubin,the patients in the observation group were divided into three groups:light,moderate and severe jaundice.At the same time,35 volunteers who received health examination from the Hongqi Hospital Affiliated to Mudanjiang Medical University and the Second Affiliated Hospital of Mudanjiang Medical University were selected as the control group.All the subjects were examined by real-time ultrasound elastography,and the correlation data were analyzed,and analyzed the diagnostic value of real-time ultrasound elastography in obstructive jaundice-induced liver disease.Result:The MEAN of the observation group was significantly lower than that of the control group,AREA% was significantly higher than that of the control group,the differences were statistically significant(P<0.05).The MEAN values of the mild,moderate and severe jaundice group were significantly lower than those of the control group (P<0.05),and the AREA% values were significantly higher than those of the control group(P<0.05),in addition,the between comparison of observation group,the differences were statistically significant(P<0.05).The ADC values of the liver in patients with mild,moderate and severe jaundice group at different b values were significantly lower than those of the control group(P<0.05);in addition,in the observation group,with the aggravation of jaundice,the liver ADC value of the patients showed a gradual downward trend,the difference between the two groups,the differences were statistically significant(P<0.05).Conclusion:Real-time ultrasound elastography is an effective way to diagnose hepatic lesions caused by obstructive jaundice.
