[摘要] 目的 探讨胃癌患者血清白细胞介素(IL)-17、同型半胱氨酸(Hcy)水平与病理特征的关系及临床诊断价值。 方法 选取河北省唐山市人民医院2015年3月—2017年3月收治的胃癌患者80例为胃癌组,另选择胃良性病变患者80例为良性病变组,同期健康体检志愿者80名为正常对照组。比较各组血清IL-17、Hcy水平,分析胃癌患者血清IL-17、Hcy與病理特征的关系,绘制ROC曲线评价IL-17、Hcy联合检测对胃癌预测价值。结果 胃癌组血清IL-17、Hcy水平高于良性病变组及对照组,良性病变组血清IL-17、Hcy水平高于正常对照组(P < 0.05);胃癌患者血清IL-17、Hcy水平与浸润深度、淋巴结转移、TNM分期有关(P < 0.05);IL-17与Hcy联合检测的曲线下面积、敏感度、特异度及约登指数均高于IL-17、Hcy 单独检测结果(P < 0.05)。 结论 IL-17、Hcy在胃癌患者血清中具有高表达,均与胃癌浸润深度、淋巴结转移及TNM分期有关,IL-17、Hcy联合检测可提高早期胃癌的诊断效果。

[Abstract] Objective To explore the relationship between serum interleukin (IL)-17, homocysteine (Hcy) levels and pathological features in patients with gastric cancer and its clinical diagnostic value. Methods A total of 80 patients with gastric cancer admitted to Tangshan People′s Hospital from March 2015 to March 2017 were selected as gastric cancer group, another 80 patients with benign gastric lesions were selected as the benign lesion group, and 60 volunteers with health examination during the same period were selected as the normal control group. The serum IL-17 and Hcy levels in each group were compared, and the relationship between serum IL-17 and Hcy and pathological features in gastric cancer patients was analyzed. ROC curve was drawn to evaluate the predictive value of combined IL-17 and Hcy detection for gastric cancer. Results The serum IL-17 and Hcy levels in gastric cancer group were higher than those in the benign lesion group and the normal control group, and serum IL-17 and Hcy levels in the benign lesion group were higher than those in the normal control group (P < 0.05). The serum IL-17 and Hcy levels in the gastric cancer patients were related to infiltration depth, lymph node metastasis and TNM stage (P < 0.05). The area under the curve, sensitivity, specificity and Youden index of IL-17 combined with Hcy detection were all higher than those of IL-17 detection and Hcy detection separately (P < 0.05). Conclusion IL-17 and Hcy are highly expressed in the serum of patients with gastric cancer, which are related to the depth of invasion, lymph node metastasis and TNM stage of gastric cancer. Combined detection of IL-17 and Hcy can improve the diagnosis effect of early gastric cancer.
