摘 要 目的:探討丁苯酞软胶囊联合奥拉西坦注射剂治疗急性脑梗死的预后效果。方法:纳入2017年8月至2019年7月江西省上饶市广信区人民医院神经内科收治的急性脑梗死患者80例,根据随机数字表法分为观察组和对照组各40例。对照组给予奥拉西坦注射剂4 g+0.9%氯化钠注射液100 ml静脉滴注,1次/d;观察组在对照组基础上联合丁苯酚软胶囊口服,200 mg/次,3次/d。治疗14 d后,比较神经功能缺损(NIHSS)评分、Barthel指数(BI)评分、肢体运动功能(FMA)评分和认知功能评分变化情况,观察临床疗效。结果:观察组总有效率为97.50%,高于对照组的75.00%(P<0.05);且干预后NIHSS评分较低,BI评分、FMA评分较高,同干预前以及对照组对比差异有统计学意义(P<0.05);而干预后蒙特利尔认知评估量表(MOCA)评分和韦氏记忆量表(WMS)评分均高于干预前和对照组(P<0.05)。结论:丁苯酞软胶囊联合奥拉西坦注射剂治疗脑梗死效果较好,可以改善患者的神经功能和认知功能,应用价值较高。

Evaluation of prognostic effect of butylphthalide soft capsules combined with oxiracetam injection in the treatment of cerebral infarction

ABSTRACT Objective: To explore the prognostic effect of butylphthalide soft capsules combined with oxiracetam injection in the treatment of acute cerebral infarction. Methods: From August 2017 to July 2019, 80 patients with acute cerebral infarction were enrolled in the Department of Neurology of Peoples Hospital in Guangxin District, Shangrao City, Jiangxi Province, and according to the method of random number table divided into an observation group and a control group with 40 cases each. The control group was given 4 g of oxiracetam + 0.9% 100 ml of sodium chloride injection intravenously, once a day; on the basis of the control group, the observation group was given oral butyl phenol soft capsule, 200 mg/time, 3 times/day. After 14 days of treatment, the changes of neurological deficit(NIHSS) score, Barthel index(BI) score, limb motor function(FMA) score and cognitive function score were compared to observe the clinical efficacy. Results: The total effective rate in the observation group was 97.50%, which was higher than 75.00% in the control group(P<0.05); after the intervention, in the observation group the NIHSS score was lower, the BI score and the FMA score were higher, and the differences were statistically significant compared with those before the intervention and the control group(P<0.05); the scores of the Montreal cognitive assessment scale and the Websters memory scale in the observation group after intervention were higher than those before the intervention and in the control group(P<0.05). Conclusion: Butylphthalide soft capsules combined with oxiracetam injection have a better effect in the treatment of cerebral infarction, can improve patientsneurological and cognitive functions, and have high application value.
