浓缩生长因子纤维蛋白治疗牙龈退缩疗效评价及对牙龈成纤维细胞增殖的影响

[摘要]目的:探討浓缩生长因子纤维蛋白(Concentrated growth factor fibrin,CGF)治疗牙龈退缩的效果及对牙龈成纤维细胞(Human gingival fibroblast,HGF)增殖的影响。方法:选择2018年6月-2019年6月笔者医院收治的30例牙龈退缩患者为研究对象,采集患者静脉血制备CGF膜,将CGF膜固定于术区进行治疗,观察术前及术后6个月的牙周袋深度(Pocket depth,PD)、退缩高度(Heigh of recession,HR)、角化龈宽度(Keratinized mucosa width,KMW)、红色美学评分(Pink aesthetic score,PES)和白色美学评分(White aesthetics score,WES)。取正常牙龈组织,采用贴壁法传代培养成HGF,在电镜下观察CGF膜上HGF的黏附情况,采用CCK-8法检测HGF的增殖情况。结果:与术前比较,术后6个月患者的PD[(2.89±0.57)mm vs (2.58±0.36)mm]、KMW[(3.99±0.68)mm vs (3.55±0.64)mm]、PES[(7.42±1.63)分 vs (2.80±0.59)分]、WES[(7.90±1.74)分 vs (2.77±0.55)分]均明显增高(P<0.05),HR[(1.95±0.56)mm vs (2.97±0.48)mm]明显降低(P<0.05)。通过电镜能够清晰看到HGF呈层层黏附于CGF膜上;HGF的增殖检测结果显示,培养1d、4d、7d时,CGF组的OD值[(0.54±0.08) vs (0.34±0.07)]、[(0.85±0.12) vs (0.55±0.14)]、[(1.13±0.30) vs (0.66±0.18)]均明显高于对照组(P<0.05)。结论:CGF治疗牙龈退缩的临床效果及美学效果均显著,能够明显促进HGF的黏附、生长和增殖。

Curative Effect of Concentrated Growth Factor Fibrin in the Treatment of Gingival Recession and its Impact on Proliferation of Human Gingival Fibroblasts

Abstract:Objective  To discuss the curative effect of concentrated growth factor fibrin(CGF) in the treatment of gingival recession and its impact on proliferation of human gingival fibroblasts(HGF). Methods  30 patients with gingival recession who was taken as research objects in our hospital from June 2018 to June 2019. Venous blood was collected to prepare CGF membrane, which was fixed in the operative area for treatment. The pocket depth(PD), heigh of recession(HR), keratinized mucosa width(KMW), pink aesthetic score(PES),white aesthetics score(WES) was observed before operation and 6 months after operation. The adhesion of HGF on CGF membrane was observed under electron microscopy. The proliferation of HGF was detected by CCK-8 method. Results Compared to before operation, the PD[(2.89±0.57)mm vs (2.58±0.36)mm],KMW[(3.99±0.68)mm vs (3.55±0.64)mm], PES[(7.42±1.63)scores vs (2.80±0.59)scores], WES[(7.90±1.74)scores vs (2.77±0.55)scores] of patients were significantly increased and the HR[(1.95±0.56)mm vs (2.97±0.48)mm] of patients were significantly decreased 6 months after operation(P<0.05). By means of electron microscopy, it was clear that HGF adhered to CGF membranelayer by layer. The results of proliferation detection of HGF showed, cultured for 1d, 4d and 7d, the OD value[(0.54±0.08) vs (0.34±0.07)], [(0.85±0.12) vs (0.55±0.14)], [(1.13±0.30) vs (0.66±0.18)] of CGFgroup was significantly higher than that of control group(P<0.05). Conclusion The clinical and aesthetic effects were significant of CGF in treatment of gingival recession. CGF could significantly promote the adhesion, growth, proliferation of HGF.

膜龈术式繁多,其最终目的都是为了获取完全的根面覆盖和良好的美观效果,国内外学者将不同术式联合不同的生物材料,辅助于促进牙龈软组织的愈合[10]。人牙龈成纤维细胞是人牙龈结蹄组织中的主要细胞,主要存在于牙龈固有层中,其主要功能是合成胶原纤维、弹性纤维及基质,具有较活跃的自身更新能力,在牙周软组织的自身修复及创伤愈合中起着至关重要的作用[15]。有研究[16]分析两种富血小板纤维蛋白产物对于牙龈成纤维细胞的影响,结果表明富血小板纤维蛋白膜对于HGF增殖有促进作用。本研究还通过体外实验探讨了CGF对HGF增殖的影响。结果显示,通过电镜能够清晰看到HGF呈层层黏附于CGF膜上,HGF的增殖检测中,培养1d、4d、7d时,CGF组的OD值均明显高于对照组。说明CGF体外促进HGF的效果较好,并证实了CGF在促进HGF的增殖中起重要作用。

综上所述,CGF具有来源丰富、高效安全,无排斥反应等优点。CGF治疗牙龈退缩的临床效果及美学效果均显著,能够明显促进HGF的黏附、生长及增殖,这一结果对临床应用CGF辅助于牙龈促进软组织再生提供了重要的理论依据。
