牙周根管超声治疗仪应用于慢性牙周炎的临床疗效

摘要：目的  探讨牙周根管超聲治疗仪治疗慢性牙周炎患者的临床疗效。方法  选择2016年7月～2018年10月东莞市石碣医院口腔门诊收治的慢性牙周病患者168例，随机分为观察组和对照组，每组84例。对照组给予传统的刮治术进行治疗，观察组给予牙周根管超声治疗仪进行治疗，对比两组的临床疗效及牙周状况。结果  观察组的治疗有效率（94.05%）高于对照组（78.57%），差异具有统计学意义（P<0.05）;治疗后两组患者的牙周状况指标（PI，BI，PD，CAL）均较治疗前降低，且观察组均较对照组减轻，差异有统计学意义（P<0.05）。结论  根管超声治疗仪治疗慢性牙周炎患者可有效控制龈下牙石所致的细菌感染，对促进慢性牙周炎牙周组织愈合具有显著疗效。

Abstract：Objective  To investigate the clinical efficacy of periodontal root canal ultrasound therapy in the treatment of patients with chronic periodontitis. Methods  A total of 168 patients with chronic periodontal disease admitted to the oral clinic of Shijie Hospital of Dongguan City from July 2016 to October 2018 were randomly divided into observation group and control group， with 84 cases in each group. The control group was treated with conventional curettage. The observation group was treated with periodontal root canal ultrasound therapy. The clinical efficacy and Asian status of the two groups were compared. Results  The effective rate of treatment in the observation group （94.05%） was higher than that in the control group （78.57%）， the difference was statistically significant （P<0.05）. The periodontal status index （PI， BI， PD， CAL） of the two groups after treatment. Both were lower than before treatment， and the observation group was less than the control group， the difference was statistically significant （P<0.05）. Conclusion  Root canal ultrasound therapy for chronic periodontitis can effectively control bacterial infection caused by subgingival calculus and has a significant effect on promoting periodontal tissue healing in chronic periodontitis.

慢性牙周炎（chronic periodontitis）是由牙菌斑而引发的炎症性疾病，是发病率较高的口腔疾病。据世界卫生组织的流行病学调查资料显示，约70%～80%的成年人罹患牙周炎，其导致牙齿脱落占全球人口的 5%～15%，因此重度牙周炎已经成为全球最重要的口腔健康负担之一[1-3]。目前临床对牙周炎主要采用龈上洁治、龈下刮治及根面平整等基础治疗术，传统的刮治术操作难度大，费时费力，且容易对患者造成疼痛和牙骨质损伤。研究显示超声根管技术弥补了传统治疗的缺陷，通过超声的热效应、声流效应与空穴效应快速、有效的清除坏死及细菌，有助于降低牙周炎并发症的发生[4，5]。本研究采用根管超声治疗仪治疗慢性牙周炎患者，取得了较满意的疗效，现报告如下。

1资料与方法

1.1一般资料  选择2017年7月～2018年10月来东莞市石碣医院口腔门诊收治的慢性牙周病患者168例，男 82例，女86例，年龄21～61岁。入选对象均符合牙周炎诊断标准[6];排除近3个月内做过牙周治疗;近1个月有抗生素、非甾体抗炎药及免疫抑制剂等药物服用史，孕妇或哺乳期妇女。通过东莞市石碣医院伦理委员会审查，所有研究对象均知情同意。按随机数字表分成观察组和对照组，每组84例，两组患者的一般资料比较，差异无统计学意义（P>0.05），具有可比性，见表1。
