[摘要]目的:本研究旨在采用半張脸(Split-face)模式自身对照观察点阵CO2激光结合强脉冲光治疗中国人群的光老化皮肤临床效果与安全性。方法:选择2019年3月-2019年12月笔者医院整形美容科门诊就诊的28例Fitzpatricks Ⅲ和Ⅳ型皮肤的中国女性就医者为研究对象。采用随机数字表法选取一侧面颊部给予单独治疗(强脉冲光治疗3次),另一侧给予联合治疗(强脉冲光治疗2次,点阵CO2激光治疗1次)。在治疗前和治疗后30d评估患者的皮肤状况,包括含水量、经表皮失水率、弹性、斑点、紫外线光斑、红色区域、棕色斑、皱纹、肤质、毛孔孔径,以及不良反应。结果:与单独强脉冲光治疗相比较,点阵CO2激光联合强脉冲光可显著增加皮肤弹性、减小毛孔孔径、改善皱纹和肤质(分别为P=0.004、P=0.039、P=0.015及P=0.035)。两种治疗方法在改善光老化引起的色素沉着方面效果相似。联合治疗不损害表皮的屏障功能,治疗后无感染、色素减退或瘢痕形成。点阵CO2激光治疗后1个月发生炎症后色素沉着,在所有疗程结束后30d消失。结论:点阵CO2激光结合强脉冲光治疗中国人群的光老化皮肤优于单用强脉冲光,安全性好。

Study on the Clinical Effect and Safety of Carbon Dioxide Lattice Laser Combined with Intense Pulsed Light in the Treatment of Photoaged Skin in Chinese Population

Abstract: Objective The purpose of this study is to observe the clinical effect and safety of carbon dioxide dot-matrix laser combined with intense pulsed light on the photoaging skin of the Chinese population by using a split-face mode. Methods This study included 28 Chinese female patients with Fitzpatricks Ⅲ and Ⅳ skin who visited theThe Department of Aesthetic Plastic Surgery of our hospital from March to December 2019. The patient's random one side of the cheek was treated with single treatment (3 times of intense pulsed light treatment), and the other side was treated with combined treatment (2 times of intense pulsed light treatment and 1 time of carbon dioxide dot laser treatment). The patient's skin condition was evaluated before treatment and 30 days after treatment, including water content, transepidermal water loss, elasticity, spots, ultraviolet spots, red areas, brown spots, wrinkles, skin texture, pore size, and adverse reactions. Results Compared with intense pulsed light treatment alone, CO2 dot-matrix laser combined with intense pulsed light can significantly increase skin elasticity, reduce pore size, and improve wrinkles and skin texture (P=0.004, 0.039, 0.015, and 0.035, respectively). The two treatments have similar effects in improving pigmentation caused by photoaging. The combined treatment does not damage the barrier function of the epidermis, and there is no infection, hypopigmentation, or scarring after treatment. Post-inflammatory pigmentation occurred 1 month after CO2 laser treatment and disappeared 30 days after treatment. Conclusion CO2 dot-matrix laser combined with intense pulsed light is better than the strong pulsed light alone in treating Chinese people's photoaging skin, and it is safe.
